Hubungan Penaksiran Skor CHA2DS2-VA terhadap Readmisi Pasien Fibrilasi Atrium Non-Valvular

Abstract: Readmission rates in atrial fibrillation patients remain a high burden on the healthcare system. The CHA2DS2-VA score, an inclusive and practical clinical tool for predicting stroke, also has the potential for predicting readmission rates in atrial fibrillation patients. This study aimed to analyze the correlation between CHA2DS2-VA score assessment and the number of re-admissions within 180-day observation periode after the first admission of non-valvular atrial fibrillation patients. This study employed a retrospective cohort study design using data from the medical records of atrial fibrillation patients at Prof. Dr. R. D. Kandou Hospital from January to December 2024. The results showed that of the 143 study subjects, 30 patients (21%) experienced readmission and 113 patients (79%) did not experience readmission at 180 days after initial admission. Statistical analysis using Spearman’s rank test showed a statistically significant relationship between the CHA2DS2-VA score and the number of readmissions within 180 days (p=0.000). The relationship observed was a positive linear correlation with a moderate level (r=0.313). In conclusion, there is an increase of the 180-day readmission rate in patients with non-valvular atrial fibrillation as the CHA2DS2-VA score increases.

Abstrak: Tingkat readmisi pada pasien fibrilasi atrium masih menjadi beban tinggi bagi sistem kesehatan. Skor CHA2DS2-VA, sebagai alat ukur klinis yang inklusif dan praktis untuk memrediksi kejadian stroke juga memiliki potensi dalam memrediksi kejadian readmisi pada pasien fibrilasi atrium. Penelitian ini bertujuan untuk menganalisis hubungan antara penaksiran skor CHA2DS2-VA terhadap banyaknya readmisi dalam periode observasi 180 hari setelah admisi awal pada pasien fibrilasi atrium non-valvular. Jenis penelitian ialah studi kohort retrospektif menggunakan data rekam medis pasien fibrilasi atrium di RSUP Prof. Dr. R. D. Kandou periode Januari hingga Desember 2024. Hasil penelitian mendapatkan bahwa dari 143 subjek penelitian, terdapat 30 pasien (21%) yang mengalami readmisi, dan 113 pasien (79%) yang tidak mengalami readmisi pada pengamatan 180 hari sejak admisi awal. Uji Spearman’s rank menunjukkan terdapat hubungan bermakna antara skor CHA2DS2-VA dan jumlah kejadian readmisi dalam periode pengamatan 180 hari (p=0,000). Hubungan yang diamati berupa korelasi linear positif dengan tingkatan sedang (r=0,313). Simopulan penelitian ini ialah terdapat peningkatan kejadian readmisi dalam periode pengamatan 180 hari dan 90 hari pada pasien fibrilasi atrium non-valvular seiring dengan meningkatnya skor CHA2DS2-VA.
