Analisis Ekspresi MMP-9 dan Hitung Fibroblas sebagai Marker Penyembuhan Ulkus Traumatikus dengan St. John’s Wort (Hypericum perforatum) Topikal pada Tikus Diabetes Melitus

Abstract: Diabetes mellitus impairs oral ulcer healing by reducing fibroblast proliferation and increasing MMP-9 expression. St. John’s Wort exhibits anti-inflammatory activity and supports tissue regeneration. This study aimed to evaluate the effect of topical St. John’s Wort on MMP-9 expression and fibroblast count in the oral mucosa of diabetic rats with traumatic ulcers. Fifteen Wistar rats induced with alloxan to establish diabetes were subjected to ulcer creation on the labial mucosa. Wistar rats were divided into three groups, namely traetment group (topical St. John’s Wort oil), positive control group (Aloclair gel), and negative control group. Tissue samples were collected on day-7 for histological (HE stain) and immunohistochemical (IHC) examination. Data were analyzed using one-way ANOVA with a significance level of p<0.05. The results demonstrated that topical St. John’s Wort significantly increased fibroblast count and reduced MMP-9 expression. In conclusion, topical St. John’s Wort has the potential to accelerate traumatic ulcer healing under diabetic conditions

Abstrak: Diabetes melitus menghambat penyembuhan ulkus mulut melalui penurunan proliferasi fibroblas dan peningkatan MMP-9. St. John’s wort diketahui memiliki aktivitas antiinflamasi dan mendukung proses regenerasi jaringan. Penelitian ini bertujuan mengevaluasi efek St. John’s wort topikal terhadap ekspresi MMP-9 dan jumlah fibroblas pada mukosa mulut tikus diabetes dengan ulkus traumatik. Lima belas ekor tikus wistar diinduksi diabetes dengan alloxan, dibuat ulkus pada mukosa labial, dan selanjutnya dibagi menjadi tiga kelompok, yaitu kelompok perlakuan (St. John’s Wort oil topikal), kontrol positif (Aloclair gel), dan kontrol negatif (tidak diberikan apapun). Jaringan mukosa ulkus diambil pada hari ke-7 untuk pemeriksaan histologik (pulasan HE) dan imunohistokimia (IHC). Analisis data menggunakan one-way ANOVA dengan taraf signifikansi p<0,05. Hasil penelitian mendapatkan bahwa pemberian St. John’s wort meningkatkan jumlah fibroblas dan menurunkan ekspresi MMP-9 secara bermakna. Simpulan penelitian ini ialah St. John’s wort topikal berpotensi mempercepat penyembuhan ulkus traumatikus pada kondisi diabetes melitus.
