Diagnosis dan Manajemen Cheilitis Exfoliatif Kronis yang Diinduksi oleh Stres, Depresi, dan Kecemasan: Laporan Kasus

Abstract: Exfoliative cheilitis is a chronic inflammation of the superficial layer characterized by hyperkeratosis and desquamation of the vermilion of the lips. The causes of exfoliative cheilitis vary, inter alia stress disorders, bad habits such as licking and biting the lips, disorders of thyroid vitamin B12 deficiency, poor oral hygiene, oral candidiasis, and most often young women with a history of psychological disorders. We reported a 22-year-old woman came to the department of medicine with complaints of chapped, dry, and bleeding lips for six months which worsened when using lipstick. The patient admitted that she experienced stress due to college and denied any systemic diseases and comorbidities. Examination of the DASS-42 checklist, prescription of chlorine dioxide dental gel, and referral to a psychologist were carried out as the treatment plan. The treatment carried out for approximately one and a half months showed satisfactory results. In conclusion, this case report presents the importance of proper diagnosis, treatment planning, and education in patients with exfoliative cheilitis caused by stress, depression, and anxiety.

Abstrak: Chelitis eksfoliatif merupakan peradangan kronik pada lapisan superfisial yang dikarakteristikkan dengan adanya hiperkeratosis dan deskuamasi pada vermilion bibir. Penyebab dari chelitis exfoliatif bervariasi seperti adanya gangguan stress, kebiasaan buruk seperti menjilat dan mengigit bibir, gangguan tiroid, defisiensi vitamin B12, kebersihan mulut yang buruk, kandidiasis oral, dan yang paling sering yaitu wanita muda dengan riwayat gangguan psikologis. Seorang perempuan 22 tahun datang ke departemen penyakit mulut dengan keluhan bibir pecah-pecah, kering dan berdarah sejak 6 bulan yang lalu yang memburuk saat menggunakan lipstik. Pasien mengaku mengalami stress akibat perkuliahan dan menyangkal adanya penyakit sistemik dan komorbid. Pemeriksaan checklist DASS-42, peresepan obat oles topikal, dan perujukan dilakukan sebagai rencana perawataan. Perawatan yang dilakukan kurang lebih satu bulan setengah menunjukkan hasil yang memuaskan. Laporan kasus ini menyajikan pentingnya tepat diagnosis, rencana perawatan, dan edukasi pada pasien yang mengalami chelitis eksfoliatif yang diinduksi stress, depresi, dan kecemasan.
