PEMANFAATAN CLINICAL HANDOVER TERSTANDAR SEBAGAI STRATEGI UNTUK MENJAGA KESINAMBUNGAN INFORMASI PASIEN PPOK: STUDI KASUS

Background: Continuity of care ensures that healthcare professionals understand the needs of COPD patients, including symptom management, exacerbation handling, discharge planning, and education. The handover process, particularly using the SBAR method in nursing care, is crucial for maintaining this continuity. Objective: to explore the use of standardized clinical handover as a strategy for ensuring the continuity of information for COPD patients. Method: The study employed a qualitative case study method, including descriptive and observational approaches. Handover observations were conducted using the SBAR tool over three days, with a total of 18 observations from two patients. Results: Results indicated that SBAR communication (situation, background, assessment, recommendation) achieved 100% optimal results in the Situation and Background stages, while the Assessment stage was 67% optimal and 33.33% not optimal. In the Recommendation stage, 33.33% was optimal and 67% not optimal. Inaccuracies in SBAR communication during handover can impact the continuity of care for COPD patients from admission to discharge. Conclusion: Overall, the implementation of SBAR communication in this case study was not optimally achieved. It is recommended to establish a Standard Operating Procedure (SOP) for handover using the SBAR method to enhance nurses' motivation for systematic and comprehensive handovers
