Abstract

Physicians occupying managerial positions face the challenge of balancing organizational business imperatives with the professional values of medicine. Their dual role as clinicians and managers often creates dilemmas between patient-centered care, operational efficiency, and the financial sustainability of healthcare organizations. Although the concept of physician leadership has received increasing attention, studies specifically addressing the balance between business imperatives and medical professionalism remain limited. This study aimed to identify the strategies, decision-making factors, and implications of balancing business imperatives with medical professional values among physician-managers. An integrative review was conducted by searching the PubMed, Scopus, and ScienceDirect databases. Of the 2,867 articles initially identified, 10 met the inclusion criteria and were included in the final synthesis. The findings indicate that physician-managers reconcile business demands and professional values through continued clinical engagement, collaborative leadership, and hybrid leadership roles. Their ability to achieve this balance is influenced by managerial competencies, professional identity, organizational structure, and leadership development programs. Physician leadership was associated with improved quality of care and patient satisfaction, although its impact on organizational financial performance remained inconsistent.
