REHABILITASI MEDIK PADA PASIEN DEWASA DENGAN CEDERA PLEKSUS BRAKIALIS
Abstract
Cedera pada pleksus brakialis dapat menyebabkan disabilitas fisik yang berat. Lesi ini dapat menyebabkan hilangnya kemampuan fungsional anggota gerak atas, nyeri yang mengganggu, stres psikologis, kesulitan sosioekonomi dan mengurangi kualitas hidup pasien secara umum. Prevalensi cedera pleksus brakialis tertinggi pada usia dewasa muda 19 – 34 tahun dengan angka kejadian pada laki –laki sebesar 89%.
Penyebab tersering cedera pleksus brakialis adalah trauma, dapat disebabkan oleh berbagai mekanisme, meliputi luka tembus, terjatuh, serta kecelakaan kendaraan bermotor. Data mengenai insiden trauma plekus brakialis sulit diketahui dengan pasti, Goldie dan Coates melaporkan 450-500 kasus cedera supraklavikular tertutup terjadi setiap tahun di Inggris. Lesi traumatik yang berhubungan dengan paralisis pleksus brakialis antara lain fraktur klavikula (10%), fraktur humerus (10%), sublukasi cervical spine (5%), trauma medula spinalis cervical (5-10%).
          Penatalaksanaan pasien dengan cedera pleksus brakialis merupakan masalah kompleks yang memerlukan kerjasama yang erat dari sebuah tim terdiri dari dokter-dokter ahli dari bagian yang berbeda dan diperlukan juga kolaborasi dengan bidang lain seperti okupasional terapis, fisioterapis, psikolog, pekerja sosial dan konselor vokasional.
          Prognosis dari cedera pleksus brakialis bervariasi dan tergantung dari letak, derajat kerusakan saraf dan kecepatan memperoleh terapi. Pasien yang tidak mendapatkan penanganan yang tepat dapat memperburuk kondisinya, dengan adanya kontraktur sendi, subluksasi sendi bahu serta bertambahnya kelemahan dan atrofi otot akibat disuse.
