PENGARUH LATIHAN OTOT INSPIRASI METODE PRESSURE THRESHOLD LOADING TERHADAP MAXIMAL INSPIRATORY PRESSURE DAN KUALITAS HIDUP PASCA PENYAPIHAN VENTILASI MEKANIK

Abstract

Abstract: Introduction : Inspiratory muscle weakness after weaning from mechanical ventilation in intensive care unit patients if not treated quickly has the potential to cause respiratory failure which can lead to death. Inspiratory muscle training aims to increase strength and endurance by applying a resistive load to the inspiratory muscles to achieve a training effect. Methods: This study was a quasi-experimental study with one group pretest and posttest design which was conducted on patients in the Intensive Care Unit (ICU) after weaning from mechanical ventilation which was consulted to the Medical Rehabilitation Department of Prof. Dr. R. D Kandou Manado Hospital from October to December 2022. Subjects were taken by consecutive sampling and were given inspiratory muscle exercise therapy with the pressure threshold loading method at a dose of 1 session/day for 5 consecutive days, with an intensity on the first day 30% of the initial Maximal Inspiratory Pressure (MIP) and on the second to fifth day it was increased by 1-2 cm H2O every day, active interval time 60-90 seconds and 1-5 minutes rest interval, exercise volume 5 sets of 6 breaths in one training session. Results: After the inspiratory muscle training using the pressure threshold loading method, there was a significant increase in MIP (t = -16.733 with a p value <0.001) and improvements in quality of life on the five dimensions of health problems and the EQ-5D VAS (z = -3.482 with a value p<0.001). Conclusion: Inspiratory muscle training using the pressure threshold loading method can improve MIP and quality of life in post-mechanical ventilation patients.
