Perbedaan Kadar HDL dan Trigliserida antara Penderita Ulkus Diabetik dan Tanpa Ulkus Diabetik pada Pasien DM Tipe 2 di RSUP Prof. Dr. R. D. Kandou Manado
Abstract
Abstract: The number of people with diabetes mellitus (DM) is increasing every year, as well as the risk of its chronic complications inter alia diabetic ulcer. Peripheral artery disease caused by atherosclerosis is one of the major risk factors for developing diabetic ulcer, and low HDL is associated with atherosclerosis. High triglyceride increases the risk for neuropathy, a diabetic ulcer risk factor. This study was aimed to analyze the difference in HDL and triglyceride levels among type 2 DM (T2DM) patients with and without diabetic ulcers. This was a descriptive comparative study with a case control design. Data were secondarily obtained from patients in the period of January 1st to September 30th 2017. Purposive technique sampling was applied to attain 30 T2DM patients with ulcers and 30 T2DM patients without ulcers. Data were analyzed by using independent t-test and P <0.05 was determined as statistically significant level. The results showed that the mean HDL level in T2DM patients with diabetic ulcers was 20.47 mg/dl, lower than of T2DM patients without diabetic ulcers with a mean HDL level of 32.33 mg/dl (P=0.000). The mean triglyceride level in T2DM patients with diabetic ulcers was 150.43 mg/dl, higher than of T2DM patients without diabetic ulcers with a mean triglyceride level of 121.16 mg/dl (P= 0.141). Conclusion: There was a statistically significant difference in the mean level of HDL between T2DM patients with and without ulcers. There was no statistically significant difference in the mean level of triglyceride between T2DM patients with and without diabetic ulcers.

Abstrak: Jumlah penderita penyakit diabetes melitus (DM) setiap tahun meningkat, diikuti peningkatan risiko terjadinya komplikasi kronik, salah satunya ialah ulkus diabetik. Penyakit arteri perifer yang disebabkan oleh proses aterosklerosis merupakan salah satu faktor risiko terjadinya ulkus diabetik, dan kadar HDL dianggap berhubungan dengan proses aterosklerosis. Kadar trigliserida yang tinggi meningkatkan risiko kejadian neuropati yang merupakan salah satu faktor risiko terjadinya ulkus. Penelitian ini bertujuan untuk menganalisis perbedaan kadar HDL dan trigliserida pada pasien DM tipe 2 (DMT2) dengan dan tanpa ulkus. Jenis penelitian ialah deskriptif komparatif dengan desain kasus kontrol (case control) menggunakan data pasien yang berobat mulai 1 Januari sampai 30 September 2017. Digunakan teknik purposive sampling untuk mendapatkan sampel sebanyak 30 pasien DMT2 dengan ulkus diabetik dan 30 pasien DMT2 tanpa ulkus diabetik sebagai pembanding. Analisis penelitian ini menggunakn uji t-tidak berpasangan, dan dinyatakan bermakna jika P < 0,005. Hasil penelitian mendapatkan rerata kadar HDL pada pasien DMT2 dengan ulkus diabetik ialah 20,47 mg/dl, lebih rendah dibandingkan pasien tanpa ulkus dengan rerata 32,33 mg/dl (P=0,000). Rerata kadar trigliserida pada pasien DMT2 dengan ulkus diabetik ialah 150,43 mg/dl, lebih tinggi dibandingkan pasien tanpa ulkus dengan rerata 121,16 mg/dl (P=0,141). Simpulan: Terdapat perbedaan bermakna rerata kadar HDL antara penderita DMT2 dengan dan tanpa ulkus diabetik. Tidak terdapat perbedaan bermakna rerata kadar trigliserida antara penderita DMT2 dengan dan tanpa ulkus diabetik.
