Abstract

Medical records are important documents created by medical personnel in primary health facilities, containing patient data and a history of conditions, diseases, and treatments that have been carried out. As a vital component in all health institutions, including Puskesmas—small to medium-scale public health service facilities—efficient management of medical records is essential to ensure accuracy and ease of access. However, Puskesmas Remboken still uses a manual or traditional medical record system, which causes various obstacles in storing, searching, and preserving long-term data. To overcome this problem, digitizing medical records is an important solution to maintain the security and efficiency of storing patient history for a long time. The development of a digital medical record archive system allows health workers to enter and access patient data more accurately and quickly. This study uses the CodeIgniter framework in system development, by implementing the Waterfall methodology to ensure a structured and systematic process. Based on the analysis of user needs (medical personnel) and administrators, a functional and easy-to-use system was designed. As a result, the developed application successfully improved medical record management by reducing errors and accelerating data access. Some of the main benefits obtained include: better data accuracy by avoiding handwriting errors and lost documents, real-time patient history search, secure and durable storage without the risk of physical damage as with paper documents, and administrative efficiency by reducing manual work. The implementation of this system proves that digital transformation at Remboken Health Center is able to significantly improve medical record management, thus supporting better health services.
