Faktor Prognostik pada Efusi Pleura Ganas Berdasarkan Skor LENT di RSUP Prof. Dr. R. D. Kandou Manado

Abstract: As a complication of cancer, malignant pleural effusion (MPE) is related to varied life expectancy. LENT score is the first validated score in malignant pleural effusion. This study aimed to obtain factors affecting the MPE based on LENT score at Prof. Dr. R. D. Kandou Hospiptal, Manado. This was a prospective study with a cross sectional design Subjects were patients with MPE admitted to Prof Dr. R. D. Kandou from January to November 2022. The results obtained 30 MPE patients consisting of 50% males and 50% females with a mean age of 52 years. There were 22 patients with LDH level <1,500 IU/L dan eight patients with LDH level >1,500 IU/L. Based on ECOG score, there were three patients with score 1, seven patients with score 2, and 20 patients with score 3-4. There were 13 patients with NLR <9 and 17 patients with NLR >9. Lung cancer was the most common cancer causing MPE, followed by gynaecological cancer and breast cancer. Based on LENT score, there were 20 patients with high risk, 10 patients with moderate risk; no patients with low risk. In conclusion, LENT score can be used as prognostic score in patients with malignant pleural effusion.

Abstrak: Efusi pleura ganas (EPG) merupakan komplikasi dari kanker dan dihubungkan dengan dengan angka harapan hidup yang bervariasi. Skor LENT merupakan skor tervalidasi pertama yang digunakan pada efusi pleura ganas. Penelitian ini bertujuan untuk menggambarkan faktor-faktor yang berperan pada EPG berdasarkan skor LENT di RSUP Prof. Dr. R. D. Kandou Manado. Penelitian prospektif cross sectional dari pasien dengan EPG di RSUP Prof Dr. R. D. Kandou Manado (Januari hingga November 2022). Pada penelitian ini didapatkan 30 pasien EPG (50% laki-laki; 50% perempuan) dengan rerata usia 52 tahun. Terdapat 22 pasien dengan kadar LDH di bawah 1.500 IU/L dan delapan pasien dengan kadar LDH di atas 1.500 IU/L. Berdasarkan skor ECOG, terdapat 3 pasien dengan skor 1, 7 pasien dengan skor 2, dan 20 pasien dengan skor 3-4. Terdapat 13 pasien dengan nilai NLR di bawah 9 dan 17 pasien dengan nilai NLR di atas 9. Kanker paru merupakan tipe tumor paling sering menyebabkan efusi pleura ganas, diikuti kanker ginekologi dan kanker payudara. Berdasarkan skor LENT terdapat 20 pasien dengan resiko tinggi, 10 pasien dengan resiko sedang, dan tidak ada pasien dengan resiko rendah. Skor LENT berperan sebagai skor prognostik pada pasien dengan EPG.
