Hubungan Kadar HbA1c dengan Nilai Ankle Brachial Index pada Lanjut Usia dengan Diabetes Melitus Tipe 2

Abstract: The prevalence of type 2 diabetes mellitus (T2DM) increases with aging and unhealthy lifestyles, especially after the age of 40 years. Hyperglycemia is a risk factor for atherosclerosis. Ankle brachial index (ABI) is an option for early diagnosis of this pathological condition because it is non-invasive, low cost, and easy to perform. This study aimed to determine the relationship between HbA1c level and ankle brachial index value in elderly with T2DM. This was a quantitative study using a cross sectional approach. Samples were 30 elderly DMT2 patients at Prof. Dr. R. D. Kandou Hospital Manado during October –December 2023 that fulfilled the inclusion and exclusion criteria. Data of HbA1c were obtained from the patients’ medical records. Ankle brachial index was assessed using the Doppler ultrasound dan sphygmomanometer. The results showed a moderate and significant negative correlation between HbA1c level and ABI value (r=-0.425; p=0.019). In conclusion, there is a relationship between HbA1c level and ABI value in elderly T2DM patients. The higher the HbA1C level, the lower the ABI value.

Abstrak: Prevalensi diabetes melitus tipe 2 (DMT2) meningkat sejalan dengan proses penuaan dan gaya hidup yang tidak sehat terutma pada usia di atas 40 tahun. Hiperglikemi ialah salah satu faktor risiko terjadinya aterosklerosis. Ankle brachial index (ABI) merupakan pilihan untuk diagnosis dini pada keadaan patologi ini karena bersifat non-invasive, murah, dan mudah dilakukan. Penelitian ini bertujuan untuk mengetahui hubungan kadar HbA1c dengan nilai Ankle Brachial Index pada subjek lanjut usia (lansia) dengan DMT2. Penelitian ini menggunakan analisis kuantitatif dengan pendekatan potong lintang. Sampel penelitian ialah 30 pasien lansia DMT2 di RSUP Prof. Dr. R. D. Kandou Manado selama Oktober –Desember 2023 yang memenuhi kriteria inklusi dan eksklusi. Data HbA1c diperoleh dari data rekam medik pasien. Penilaian ABI menggunakan doppler ultrasound dan sfigmomanometer. Hasil penelitian mendapatkan korelasi negatif kekuatan sedang yang bermakna pada hubungan antara kadar HbA1c dengan nilai ABI (r=-0,425; p=0,019). Simpulan penelitian ini ialah terdapat hubungan bermakna antara kadar HbA1c dengan nilai ABI pada pasien lansia dengan DMT2. Semakin tinggi kadar HbA1c, maka semakin rendah nilai ABI.
