FAKTOR PENYEBAB MEDICATION ERROR PADA PELAYANAN KEFARMASIAN RAWAT INAP BANGSAL ANAK

ABSTRACT

Medication Error ( ME ) is a patient adverse events resulted from the use of drugs by the treatment of health personal which could otherwise be prevented. This research aims to determine ME factors in the phase of prescribing, dispensing and administration. This research is using the descriptive survey with data collection techniques through a questionnaire to the respondent doctors, nurses, pharmacists and assistant pharmacists at irina E department of Prof. Dr. R.D. Kandou Manado then the data were analyzed using univariate analysis. It’s showed that the ME factors of prescribing phase include workload that the ratio of the workload and the human resources are not balanced, education that prescription does not qualify completeness recipes, impaired work such as interrupted by phone ringing, the environmental condition such as poor lighting support at work and communication by verbal order. Factors cause ME dispensing phase includes the workload that the ratio of the workload and the human resources are not balanced, education by preparation of the drugs that wasn’t prescription demand, communication by the lack of communication regarding the stock of pharmaceuticals, environmental conditions such as no room preparation of drugs and impaired work such as interrupted by phone ringing. Factors cause ME administration phase includes the workload that the ratio of the workload and the human resources are not balanced, impaired work such as interrupted by phone ringing, education such as not timely administration of drugs, environmental conditions such as pharmaceutical unit location does not make it easier for health workers on drug delivery and communication such as the lack of communication of health workers and patients in drug use. Various factors have been identified in this study could affect the treatment of patients.

ABSTRAK

Medication Error (ME) adalah kejadian yang merugikan pasien akibat pemakaian obat selama dalam penanganan tenaga kesehatan yang sebetulnya dapat dicegah. Tujuan penelitian yaitu mengetahui faktor penyebab ME pada fase prescribing, dispensing dan administration. Jenis penelitian survei deskriptif dengan teknik pengambilan data melalui kuesioner kepada responden dokter, perawat, apoteker dan asisten apoteker yang bertugas di irina E RSUP Prof Dr. R.D. Kandou Manado kemudian data dianalisis menggunakan analisis univariat. Hasil penelitian menunjukkan bahwa faktor penyebab ME fase prescribing meliputi beban kerja yaitu rasio antara beban kerja dan SDM tidak seimbang, edukasi yaitu penulisan resep tidak memenuhi syarat kelengkapan resep, gangguan bekerja yaitu terganggu dengan dering telepon, kondisi lingkungan yaitu pencahayaan yang kurang mendukung saat bekerja, dan komunikasi yaitu permintaan obat secara lisan. Faktor penyebab ME fase dispensing meliputi beban kerja yaitu rasio antara beban kerja dan SDM tidak seimbang, edukasi yaitu penyiapan obat yang tidak sesuai permintaan resep, komunikasi yaitu kurangnya komunikasi mengenai stok perbekalan farmasi, kondisi lingkungan yaitu tidak adanya ruangan penyiapan obat dan gangguan bekerja yaitu terganggu dengan dering telepon. Faktor penyebab ME fase administration meliputi beban kerja yaitu rasio antara beban kerja dan SDM tidak seimbang, gangguan bekerja yaitu terganggu dengan dering telepon, edukasi yaitu tidak tepat waktu pemberian obat, kondisi lingkungan yaitu jarak unit farmasi tidak memudahkan tenaga kesehatan dalam pemberian obat dan komunikasi yaitu kurangnya komunikasi tenaga kesehatan dan pasien dalam penggunaan obat. Berbagai faktor yang teridentifikasi dalam penelitian ini dapat berpengaruh terhadap pengobatan pasien.
