           Drug interaction become one of the most serious problems in therapy if drug interaction affect the success of the therapy and potentially lead to therapeutic failure. The increased incidence of drugs interaction with undesirable effects is the result of more and more frequent use of drug called polypharmacy or multiple drug therapy. Patients with acute renal failure (ARF), and chronic renal failure (CRF) are often prescribed with many of drugs. Drugs with many therapeutic classes use to treat disease leading to CRF and ARF, the risk of drug interaction is greater with increasing complexity of medications, which used in treatment. This study aims to determine the potential of drugs interactions in the treatment of patients with renal failure against inpatient of Prof. DR.R.D. Kandou Hospital, Manado. This research is a descriptive study with prospective data taking on 45 medical record data of patients with renal failure inpatients who meet in the inclusion criteria. The result of research about potential drug interaction that can occur in patients with renal failure is 292 potency of drug interaction with percentage of 65,32% with severity of major potency about 22 (7,53%), moderate potency as much as 206 (70,54%) and minor potency as much as 64 (21,91%).

Interaksi obat menjadi salah satu masalah yang serius dalam terapi karena jika terjadi interaksi obat akan mempengaruhi keberhasilan terapi dan berpotensi menyebabkan kegagalan terapi, meningkatnya kejadian interaksi obat dengan efek yang tidak diinginkan adalah akibat makin banyaknya dan makin seringnya penggunaan obat - obat yang dinamakan polifarmasi atau multiple drug therapy. Pasien dengan gagal ginjal akut (GGA), dan gagal ginjal kronis (GGK) sering diresepkan banyak obat. Obat dengan banyak kelas terapi digunakan untuk mengobati penyakit yang mengarah ke GGK dan GGA, resiko terjadinya interaksi obat semakin besar dengan meningkatnya kompleksitas obat-obat yang digunakan dalam pengobatan. Penelitian ini bertujuan untuk mengetahui potensi interaksi obat pada pengobatan pasien dengan gagal ginjal di instalasi rawat inap RSUP Prof. DR. R. D. Kandou Manado. Penelitian ini merupakan penelitian deskriptif dengan pengambilan data secara prospektif terhadap 45 data rekam medik pasien gagal ginjal rawat inap yang memenuhi kriteria inklusi. Hasil penelitian tentang potensi interaksi obat yang bisa terjadi pada pasien gagal ginjal yaitu sebanyak 292 potensi interaksi obat dengan persentase 65,32%, dengan tingkat keparahan major sebanyak 22 potensi (7,53 %), moderate sebanyak 206 potensi (70,54%) dan minor sebanyak 64 potensi (21,91%).
