ABSTRACT
The use of drugs to patients with kidney failure can cause Drug Related Problems (DRPs). DRPs are any incidents involving medication therapy of patients who are actually or potentially will affect the outcome of desired therapeutic. This research aims to determine the Drug Related Problems (DRPs) in patients with renal failure in the inpatient department of Prof. DR. R. D. Kandou Manado. This study was conducted prospectively during May 2016 for inpatient B Hospital Prof. DR. R. D. Kandou Manado. This research was conducted on 27 patients with a medical record of chronic kidney disease. The results showed the number of drug-related problems are known to potentially experience DRPs are indications without treatment (3.6%), without therapy indication (0%), improper drug selection (0%), sub-therapeutic doses (36.9%), excessive drug dose (3.3%) and patients failed to receive medications (56.2%).

ABSTRAK
Penggunaan obat terhadap pasien gagal ginjal memungkinkan terjadinya Drug Related Problems (DRPs). DRPs merupakan setiap kejadian yang melibatkan terapi pengobatan pasien yang secara nyata atau potensial akan mempengaruhi hasil terapi yang diinginkan. Penelitian ini bertujuan untuk mengetahui Drug Related Problems (DRPs) pada pasien gagal ginjal di rawat inap RSUP Prof. DR. R. D. Kandou Manado. Penelitian ini dilakukan secara prospektif selama bulan Mei 2016 di instalasi rawat inap B RSUP Prof. DR. R. D. Kandou Manado. Penelitian ini dilakukan terhadap 27 catatan rekam medik pasien dengan penyakit gagal ginjal kronik. Hasil penelitian menunjukkan jumlah masalah terkait obat yang diketahui berpotensi mengalami DRPs yaitu indikasi tanpa terapi (3,6%), terapi tanpa indikasi (0%), pemilihan obat tidak tepat (0%), dosis sub terapi (36,9%), dosis obat berlebih (3,3%) dan penderita gagal menerima obat (56,2%).
