Hubungan Kadar Albumin Serum dengan Elektrolit Serum dan Marker Hematologik pada Pasien Kanker Paska Pembedahan

Abstract: Cancer patients tend to experience hypoalbuminemia that can affect body functions inter alia serum electrolyte and serum proteins levels. This study aimed to analyze the relationship between serum albumin levels and several laboratory parameters such as serum electrolytes, leukocyte count, platelets, hemoglobin. This was an analytical and observational study with a cohort design. Data collection was carried out prospectively by tracing the medical records of cancer patients who underwent surgery at Prof. Dr. R. D. Kandou Hospital Manado in the period October-December 2024. Patients who met the inclusion criteria were divided into two groups, namely patients with albumin levels <2.5 g/dl and albumin levels ≥2.5 g/dl. The relationship between serum albumin levels and laboratory parameters was analyzed using the independent t-test. The results of bivariate analysis for leukocyte parameters showed a significant difference between the mean leukocyte values in the two sample groups (p 0.045). The results of the analysis on other parameters such as serum electrolytes (Na, K, and Cl), hemoglobin levels, platelets, MCV, MCH, and MCHC did not show any significant differences (p>0.05). In conclusion, there is a significant relationship between serum albumin level and leukocyte level, namely cancer patients with hypoalbuminemia have higher leukocyte level. Therefore, the leukocyte-albumin ratio can be considered as a predictor of the clinical outcome of cancer patients undergoing surgery.

Abstrak: Penderita kanker cenderung mengalami hipoalbuminemia yang dapat memengaruhi fungsi tubuh antara lain kadar elektrolit serum dan protein serum. Penelitian ini bertujuan untuk menganalisis hubungan antara kadar albumin serum dengan beberapa parameter laboratorium seperti elektrolit serum, hitung leukosit, trombosit, dan hemoglobin pada pasien kanker yang menjalani pembedahan. Metode penelitian ialah observasional analitik dengan desain kohort. Pengumpulan data dilakukan secara prospektif dengan penelusuran data rekam medis pasien kanker yang menjalani pembedahan di RSUP Prof. Dr. R. D. Kandou Manado periode Oktober- Desember 2024. Pasien yang memenuhi kriteria inklusi dibagi menjadi dua kelompok yaitu pasien dengan kadar albumin <2,5 g/dl dan kadar albumin ≥2,5 g/dl. Hubungan antara kadar albumin serum dengan parameter laboratorium dianalisis menggunakan uji independent t-test. Hasil analisis bivariat untuk parameter leukosit menunjukkan perbedaan bermakna antara rerata nilai leukosit pada kedua kelompok sampel (p 0,045). Hasil analisis pada parameter lainnya seperti elektrolit serum (Na, K, dan Cl), kadar hemoglobin, trombosit, MCV, MCH, dan MCHC tidak menunjukkan adanya perbedaan bermakna (p>0,05). Simpulan penelitian ini ialah terdapat hubungan bermakna antara kadar albumin serum dengan kadar leukosit, yaitu pasien kanker dengan hipoalbuminemia memiliki kadar leukosit yang lebih tinggi. Rasio leukosit-albumin dapat dipertimbangkan menjadi prediktor terhadap luaran klinis pasien kanker yang menjalani tindakan pembedahan.
