Perbandingan Luaran Fungsional Pasien Pasca Rekonstruksi ACL dengan Autograft Hamstring dan Autograft Peroneus Longus

Abstract

Abstract: Anterior cruciate ligament (ACL) reconstruction aims to restore knee stability and function after injury. The choice of autograft is an important factor in influencing postoperative functional outcomes. Hamstring autografts are commonly used, but the peroneus longus is now often used as a promising alternative because of its similar tendon strength and lower donor site morbidity. This study aimed to compare the functional outcomes of patients with post ACL reconstruction using hamstring autograft and those using peroneus longus autograft. This was a comparative analytical study involving 51 patients who underwent ACL reconstruction at Prof. Dr. R. D. Kandou General Hospital with a follow-up period of ≥6 months. Functional outcomes were assessed using the IKDC, Lysholm, and ACL-RSI questionnaires and analyzed using the independent sample t-test (p<0.0167). Most patients were males aged 19–44 years with sports-related injuries (72.5%). Peroneus longus autograft (51%) showed lower donor site complications and morbidity without significant differences in IKDC (p=0.685), Lysholm (p=0.356), and ACL-RSI (p=0.653) scores. In conclusion, the peroneus longus autograft is a viable and effective alternative in ACL reconstruction procedures, as it provides functional outcomes comparable to hamstring autografts.

Abstrak: Rekonstruksi Anterior Cruciate Ligament (ACL) bertujuan memulihkan stabilitas dan fungsi lutut pasca cedera. Pemilihan autograft menjadi faktor penting dalam memengaruhi luaran fungsional pascaoperasi. Autograft hamstring umum digunakan, namun autograft peroneus longus kini sering digunakan sebagai alternatif menjanjikan karena kekuatan tendon serupa dan morbiditas donor site lebih rendah. Penelitian ini bertujuan untuk membandingkan luaran fungsional pasien pasca rekonstruksi ACL menggunakan autograft hamstring dengan autograft peroneus longus. Metode penelitian analitik komparatif yang melibatkan 51 pasien pasca rekonstruksi ACL di RSUP Prof. Dr. R. D. Kandou dengan follow-up ≥6 bulan. Luaran fungsional dinilai menggunakan kuesioner IKDC, Lysholm, dan ACL-RSI, dianalisis dengan uji independent sample t-test (p<0,0167). Hasil penelitian mendapatkan bahwa sebagian besar pasien laki-laki berusia 19–44 tahun dengan penyebab cedera akibat olahraga (72,5%). Autograft peroneus longus (51%) menunjukkan komplikasi dan morbiditas donor site lebih rendah tanpa perbedaan bermakna pada skor IKDC (p=0,685), Lysholm (p=0,356), dan ACL-RSI (p=0,653). Simpulan penelitian ini ialah autograft alternatif peroneus longus layak dan efektif dalam prosedur rekonstruksi ACL karena memberikan hasil fungsional yang sebanding dengan autograft hamstring.
